CIVIL REGISTRATION VITAL STATISTICS (CRVS)

Civil Registration is the compulsory, continuous, universal and permanent recording of vital events (birth, death, marriage and divorce). Vital statistics is the data generated as a result of civil registration record of vital events which can be used for effective policy and planning. Under Section 29(1) C of the LGA 2013 rights of Birth, Death, Marriage and Divorce is the mandate of VCs/NCs.

SITUATION ANALYSIS:

  • Almost 230 million children worldwide are not registered
  • 38 million annual deaths are not registered
  • 42% birth registration of children in Pakistan
  • 19% birth registration in Khyber Pakhtunkhwa (MICS 2017)
  • 1% in erstwhile FATA (MICS 2017)
  • Less than 5% deaths registration in Khyber Pakhtunkhwa

BIRTH REGISTRATION DRIVE 2020-21:

Keeping in view the low dismal rate, Provincial Government assign a responsibility for LG&RD to register 50% children of KP by 31/12/2020. LG&RD launched a special campaign and was able to meet the objective within the term by making door to door visits. In the first phase, 12.239M children have been registered. The aim was updated in January 2021 and it was requested to register 100% children of the province by 30/06/2021.The department registered nearly 15 million children within the time period which is 85% of the total target.

DOCUMENT REQUIRED FOR BIRTH CERTIFICATE

For the purpose of birth certificate, the applicant is required to furnish the following documents:

  1. Application form, as specified in Form-A, provided by concerned council duly filled along with signature and thumb impression;
  2. Duly attested copy of the CNIC or passport, in case of being foreigner, or residence permit, in case of a refugee, of parent(s) or guardian; and
  3. Birth certificate or immunization card, issued by health facility or school certificate, if available; and

DOCUMENT REQUIRED FOR DEATH CERTIFICATE:

The following actions must be taken by the petitioner in order to receive a death certificate:

  1. The applicant shall be bound to fill the application form, as specified in Form-D, verify its entries, put his signature and thumb impression and write thereon his Computerized National Identity Card number.
  2. The applicant may be required to provide all documentary evidences, including certificate or parchi, issued by Ghorkan of the graveyard or person in charge of the concerned graveyard, if any, as the case may be, where deceased was buried, etc.

DOCUMENT REQUIRED FOR MARRIAGE CERTIFICATE:

For the purpose of marriage certificate, the applicant is required to furnish the following documents:

  1. Application form, as specified in Form-M, provided by concerned council duly filled along with signature and thumb impression;
  2. Copy of Nikah Nama duly registered by the Marriage Registrar or the marriage certificate, in case the marriage being solemnized by any other authority, as per personal laws of non-Muslims; and
  3. Copy of CNIC of husband and wife, along with copies of their parent’s CNIC. Alternatively, the copies of passport, in case of foreigner, or residence permit, in case of refugee.

 

DOCUMENT REQUIRED FOR DIVORCE CERTIFICATE:

The application for registration of divorce must be made on prescribed Form-Q along with the necessary documents as specified in the said Form.

  1. Fill the application Form-Q;
  2. Write the date of divorce and the reason thereof and give an affidavit;
  3. Verify its entries, put his signatures and thumb impression and write thereon his National Identity Card number.
  4. Attach the following documents:
      1. Written statements; and
      1. Divorce deed or talaqnama or the order issued by the Court of Law or the Chairman of the concerned Arbitration Council or any other concerned authority in Pakistan or abroad.
